Recently, the Massachusetts Dental Board of Registration in Dentistry (BORID)
has adopted new guidelines regarding anesthesia within the dental office
setting. In order to utilize the services of McCarty Anesthesiology, LLC a
prospective dental office must acquire a "Facility Permit D" in conjunction with
the mobile facility permit issued to McCarty Anesthesiology, LLC. I have
attempted to make this process as readily accessible as possible. Below, I have
provided links to the Facility Permit Application, Attestation between McCarty
Anesthesiology, LLC and your respective office, along with an explanation of
what is required on each of our part. Please feel free to contact either myself or

Please refer to the SAS Order Form below. I have included the contact information for Southern
Anesthesia & Surgical. Due to the controlled substance (Midazolam) an account with Southern
Anesthesia needs to be set up and that requires a copy of your DEA certificate, Massachusetts
Dental License & MA Controlled Substance Registration. Customer Service can explain further. I
have suggested Southern Anesthesia but any pharmaceutical supplier will have these medications
available to you.

However, please note you would still need to purchase the following separately; SoluCortef
(corticosteroid), Midazolam (anticonvulsant), and Ephedrine (vasopressor).


Please keep in mind, these are just my suggestions. You may fulfill the requirements with other
medications and/or dosages. In the event of an adverse, emergent situation during a case where I
am administering the anesthesia, I have my own pharmaceutical supply and the medications you are
purchasing are not necessary. The BORID has stated in order for you to acquire your own facility
permit, you must meet the standards set forth for those offices utilizing nitrous oxide (Permit C). In
their mind, this would allow for emergencies to be handled by the office personnel.
Steps to Acquiring Facility Permit "D"
(1) The Facility Permit "D" Application
(2) Purchase the Required Medications, in accordance with Permit "C" Nitrous Oxide
(3) Purchase a Stethoscope & Sphygmomanometer

(4) Obtain "E" cylinder of medical oxygen, tank regulator, and a positive pressure device
I suggest you simply rent an Oxygen tank and regulator through a local distribution company. This is
both inexpensive and the maintenance is taken care of for you. You simply exchange the tank as it is

Technically, you only need an "oxygen mask". However, for the benefit of
patient safety, I reccomend having an Ambu bag in the office for any emergent
situation, wether it involves sedation or otherwise.

(5) Complete the Facility "D" Permit Application, by providing;
          
             (a.) License Information
             (b.) CPR or BLS certification of the Dentists / Assistants who will treat patients               
                     while under sedation & anesthesia
             (c.) Complete pages 1,2,4,5,6 on the application. Denote on your application                
                    you are applying for a Facility Permit "D" in conjunction with McCarty                       
                    Anesthesiology, LLC mobile facility permit. The sections regarding Permit A            
                    & Permit B (page 3) do not pertain to your application unless you wish to                 
                    administer general anesthesia or enteral sedation on your own without the              
                    presence of McCarty Anesthesiology,LLC. You are also not required to obtain        
                    Permit C (nitrous oxide administration). The BORID is only requiring the basic         
                    level of emergency equipment & supplies for use of sedation & anesthesia.
